Our Care Coordinator, Nicole is here to assist you and your family with things such as food resources, low-income housing, WIC, applications for food stamps or TANF.

If your child requires a referral to a specialist, she will initiate the referral to the appropriate clinic as well as follow up with you after to make sure that they have connected with you.

She also can also work closely with you to create an individualized care plan for your child with complex medical needs. If you have any question, concern, or resource need, please reach out to Nicole!
