To parents and guardians of kids 13 years and older, Congratulations! Your child has reached the teenage years. We want to help your teen prepare to be an active participant in his/her/their medical care. At the annual well child check, we will be administering an Adolescent Health Questionnaire as well as starting to talk to your adolescent independently for part of the visit. While the transition from childhood to adulthood can be exciting, it can present some unique challenges. Thus, it is crucial for us to both use the questionnaire and take the time to talk to your adolescent in private to help address issues that he/she/they may not necessarily be comfortable discussing with a guardian in the room.
